BRIDGE To Home provides personalized, compassionate care for individuals of all ages with disabilities, special healthcare needs, and/or mental health needs throughout Wisconsin and Minnesota. Our experienced and thoughtfully trained direct support professionals (DSPs) help individuals live more independently, build meaningful connections, and participate fully in everyday life—at home, at school, at work, and throughout their communities.

We believe great care is about more than meeting basic needs. It’s about connection, choice, independence, and belonging. We work closely with individuals and families to create an individualized care plan that reflects each person’s goals, strengths, routines, and unique needs.

We also understand that caregiving affects the entire family. The ongoing demands of caring for a loved one can impact work, relationships, and time for yourself. Services

BRIDGE To Home provides these services for children and adults at home in Wisconsin and Minnesota:

Assist with routine activities of daily living, such as dressing, feeding, and fitness.
Assist with medications.
Routine housekeeping for client.
Assist with communication.
Help client with community activities and outings.
Offer companionship and socialization.
Teach daily living skills.
Assist with home based therapy exercises.
Provide respite care and caregiver relief.
